People Incorporated Mental Health Services is a nonprofit organization based in the Twin Cities metro area of Minnesota, providing a wide range of mental health and disability services across multiple counties.
The Scott County Waiver Case Management (WCM) Team operates out of the Eagan office and serves individuals in Scott County who are enrolled in Minnesota’s disability waiver programs, including CADI, BI, DD, and EW waivers. The team works within the Community Based Services Manual (CBSM) regulatory framework and coordinates services through MnCHOICES.
The team is responsible for completing and coordinating annual and reassessment meetings with participants, developing and managing individualized support plans, and authorizing and coordinating a wide range of waiver services including CFSS, IHS, homemaking, home modifications, assistive technology, EAA, and CDCS. The team also works closely with Scott County, providers, FMS agencies, and other care stakeholders to ensure participants receive appropriate and timely services, while maintaining compliance with DHS regulations, STLC requirements, and billing standards. A core focus of the team is advocating for participants to access meaningful services and community integration opportunities.
