We have been providing person-centered support and teaching for people with intellectual and developmental disabilities in a variety of settings, for 125 years.
*We are a licensed 245D (Home and Community Based Services) by the Minnesota Department of Human Services. One of our service categories is independent living services.
*We are a Housing Supports and Housing Stabilization provider, registered with Minnesota Department of Human Services.
* We provide a Family Navigation Service to assist IDD individuals and families in accessing resources/services/supports, helping advise with life transitions, self advocacy training, and development of community networks and social integration. Services are goal-based, as identified, and approved by each person and his/her team.
For people to live their own best lives, they need to understand their rights and their responsibilities. This kind of teaching and support is embedded in client goals. Balancing safety and risk are a part of learning this. Life cannot be all about safety, and risks need to be understood and balanced with safety concerns.
We assist individuals and their families to access other community-based supports s/he/they need and want to be successful. This includes employment related connections, health related connections, therapeutic connections (OT, PT, speech/language/communication, music, and art), community organizations, educational support, social services, including benefits like social security, food shelf, etc.
