I am writing to request support for a patient who is currently experiencing homelessness and has no income. She also has significant health barriers that make communication and follow-up care especially challenging.
Access to a reliable phone is critical for her ability to:
Stay connected with healthcare providers
Receive important appointment reminders
Coordinate care and support services
At this time, she does not have a phone. We are requesting assistance in the form of a gift card (Target or Best Buy preferred) so that she may purchase a basic flip phone along with a prepaid calling card for $150.
This resource would make a meaningful difference in helping her remain engaged in her care and improve her overall stability.
